For Your Stress Test

  • Your insurance information and card (i.e., HMO, Medicare, Medicaid)

  • List of any medications that you may be taking and their dosages

  • Comfortable clothes and shoes for walking

  • Do not eat six hours before the stress test

For Invasive Out-Patient Procedures

  • Your insurance information and card (i.e., HMO, Medicare, Medicaid)

  • List of any medications that you may be taking and their dosages

  • Do not eat six hours before the procedure

For First Time Visits

  • Below is a list of items which you should bring for your first appointment with Dr. Williams at HVA. The receptionist scheduling you for your first visit will go over most of these items, but you may also want to print this list for your own reference before your visit.

  • Your insurance information and card (i.e., HMO, Medicare, Medicaid)

  • Some form of identification

  •  List of any medications that you may be taking and their dosages
    Copies of your medical history, x-rays, and/or lab reports if you have them

  • You can go online to fill out the forms

  • Any questions you may have for your doctor

  • We recommend you bring a friend or family member to your initial visit.

For Follow Up Visits

  • Your insurance information and card (i.e., HMO, Medicare, Medicaid)

  • List of any medications that you may be taking and their dosages

  • Any new tests results: x-rays, and/or lab reports

  • Any questions you may have for your doctor

  • Blood Pressure Monitor Chart

  • Blood Sugar Records
