What to bring for your first office visit:

Below is a list of items which you should bring for your first appointment with Dr. Williams at Cardiac Associates of North Jersey. The receptionist scheduling you for your first visit will go over most of these items, but you may also want to print this list for your own reference before your visit.

Your insurance information and card (i.e., HMO, Medicare, Medicaid)
List of any medications that you may be taking and their dosages
Copies of your medical history, x-rays, and/or lab reports if you have them
Any forms you were asked to bring with you (Form: Patient Medical History)
Any questions you may have for your doctor
We recommend you bring a friend or family member to your initial visit.

For Follow up visit:

Comfortable clothes and shoes for walking
Do not eat six hours before the stress test

For invasive out patient cardiac procedures:
